Learn how today’s nutrition recommendations emphasize whole foods, healthy fats, and balanced eating patterns to support better heart health and reduce your risk of cardiovascular disease.

The Food Pyramid

For decades, Americans relied on the USDA Food Pyramid as a guide to healthy eating. Bread, cereal, rice, and pasta formed the broad base, while fats and sweets occupied the tiny space at the top. It was originally introduced in 1992. There have been several relatively minor updates over the years. In 2011, the “My Plate” dietary guide was introduced, but failed to gain recognition.

Now, the U.S. Department of Health and Human Services (HHS) and USDA have taken the original food pyramid and literally turned it upside down.

The 2025–2030 Dietary Guidelines for Americans, released in January 2026, introduced a completely redesigned food pyramid that puts protein, dairy, and healthy fats near the top along with vegetables and fruits, while whole grains occupy a smaller section near the bottom. The new guidelines also put much greater emphasis on whole, nutrient-dense foods and reducing highly processed foods. (Real Food)

This is a significant change. But turning the pyramid upside down doesn’t mean we need to completely change our diets. Many principles still apply. However, some changes, particularly the emphasis on red meat and full-fat dairy, may contradict what we’ve been told for many years. These new recommendations may require some explanation for people concerned about cholesterol and heart disease.

What Changed in the New Food Pyramid?

When the original USDA Food Guide Pyramid was introduced in 1992, it placed bread, cereal, rice, and pasta at its foundation, recommending six to eleven servings per day. Fruits and vegetables occupied the next level, followed by meat and dairy. Fats, oils, and sweets sat at the very top and were recommended to be used sparingly.

Nutrition guidance has changed considerably since then.

The new 2025–2030 guidelines focus heavily on food quality rather than simply assigning foods to serving categories. The overall message is to eat nutrient-dense, unprocessed foods. Prioritize protein, eat plenty of vegetables and fruits, include whole grains, and sharply reduce foods containing large amounts of refined carbohydrates, added sugars, and highly processed ingredients. (Real Food)

One of the most important changes is the emphasis on where your food comes from and how it is prepared.

For example:

A fresh apple and a prepackaged sugary fruit snack are both sources of carbohydrates, for example, but they clearly aren’t nutritionally equivalent. Neither are oatmeal and a frosted doughnut.

The same principle applies to protein and fat.

Salmon, lentils, walnuts, bacon, and a cheeseburger may all contain protein or fat, but they have significantly different effects on cardiovascular health.

Most Notable Improvement: Less Emphasis on Refined Carbohydrates

One of the most significant changes is the movement away from recommending large quantities of grain-based foods, especially refined grains

The difference between whole and refined grains matters.

Whole grains include the bran, germ, and endosperm of the grain and provide fiber along with vitamins and minerals. Refining grains removes the bran and germ, which strips away most of the fiber and some nutrients.

The American Heart Association continues to recommend whole grains as an important part of a heart-healthy diet and recommends three or more servings of fiber-rich whole grains each day.

Fiber-rich whole grains include:

Whole wheat bread

Whole wheat pasta

Oatmeal

Brown and wild rice

Barley

Quinoa

Farro

Although the new pyramid focuses less on grains, unprocessed whole grains are still considered healthy.

Replacing refined grains and heavily processed carbohydrates with whole grains, vegetables, fruits, beans, nuts, and other nutrient-dense foods is a significant step towards improving cardiovascular health.

Protein Now Has a Much Bigger Role

Another noticeable feature of the new pyramid is the prominent position given to protein.

Protein is essential for maintaining muscle, repairing tissue, producing enzymes and hormones, and supporting numerous other functions throughout the body.

The recommendation is to eat 1.2–1.6 grams of protein per kilogram of body weight per day. When it comes to cardiovascular health, the source of your protein matters just as much as the amount.

The new recommendation is to include a variety of protein sources in your diet, such as eggs, poultry, meat, seafood, beans, peas, lentils, legumes, nuts, seeds, and soy.

Fat Is No Longer the Enemy — But the Type Matters

One of the biggest changes in nutritional science over the past several decades has been the way we look at fat in our diet. We no longer consider all dietary fat to be unhealthy.

Fat is an essential nutrient. But there are different types of fat, and it’s important to understand why some are healthy and why some are not. The primary categories referenced in the new dietary guidelines are saturated fat and unsaturated fat.

The difference is at a molecular level and in the bonds between the carbon atoms within each fat molecule. But for this article, we’ll focus on how they affect heart health.

Unsaturated fat

Lowers LDL (bad cholesterol)

Burns more energy to process

Lowers blood glucose levels

Lowers blood pressure

Reduced post-meal triglycerides

Promotes weight loss

Is usually liquid at room temperature

Saturated fat

Raises LDL (bad cholesterol)

Burns less energy to process

Raises blood glucose levels

Raises blood pressure

Raises post-meal triglycerides

Is easily stored/promotes weight gain

Is usually solid at room temperature

Foods rich in unsaturated fats include:

Olive and other non-tropical plant oils, nuts, seeds, avocados, and many types of fish.

The new pyramid may make foods such as meat and dairy appear more prominent, but that doesn’t erase decades of research surrounding LDL cholesterol.

Saturated fat can increase LDL, commonly called “bad” cholesterol. Elevated LDL contributes to the development of atherosclerosis, the buildup of plaque inside arteries that can eventually lead to heart attack or stroke.

The new dietary guidelines recommend replacing sources of saturated fat with unsaturated fats. In addition, limiting saturated fat to less than 10% of total daily calories is recommended for people aiming to lower cardiovascular risk and reduce the risk of heart attacks.

What About Red Meat?

One of the controversial aspects of the new recommendations is the inclusion of red meat.

Benefits of eating red meat:

Red meat is a good source of protein and other essential nutrients. Red meat contains proteins not found in other foods. Some nutrients, like iron, zinc, and vitamin B12 from red meat, are more easily absorbed than from other sources, making it a much better source.

Risks associated with red meat:

The problem with red meat is that many cuts can also contain substantial amounts of saturated fat. The health risks associated with eating high levels of saturated fat can easily outweigh the benefits from eating red meat.

The healthy way to include red meat in your diet:

For someone concerned about cardiovascular health, the practical lesson is not necessarily that red meat must disappear from the menu.

The solution to making red meat part of a healthy diet is easier than you may think. Choose leaner cuts and rotate your protein sources. By choosing leaner meat, it’s easier to stay within the recommended 10% daily maximum of calories from saturated fats.

Avoid processed meats such as bacon, sausage, hot dogs, and many deli meats. They have a lower nutritional value and can also contain substantial amounts of sodium.

Is Eating Full-Fat Dairy Recommended?

Yes, it is. Dairy is an excellent source of protein, healthy fats, vitamins, and minerals. It is recommended to include 3 servings per day of full-fat dairy with no added sugars.

Similar to red meat, dairy can be a source of unwanted saturated fats. Whole milk, cream, butter, and many cheeses can contribute significant amounts of saturated fat to the diet. For someone with elevated LDL cholesterol or cardiovascular risk factors, consuming large quantities may make it harder to keep saturated-fat intake under control.

It’s important to keep your daily intake of saturated fats to a maximum of 10% of your total calories consumed.

Vegetables and Fruits Promote Heart-Health

Including vegetables and fruits has always been an important part of a heart-healthy diet. They provide fiber, vitamins, minerals, and numerous other nutrients while helping create a diet that is generally lower in calories.

Rotating your sources of nutrients is an important part of a balanced diet. Eating a variety of fruits and vegetables can help ensure a balanced and nutritious diet. Instead of relying on the same vegetable every day, rotate among leafy greens, broccoli, peppers, tomatoes, carrots, squash, beans, cauliflower, and other vegetables.

Fresh fruits and vegetables are always best. They generally have the greatest nutritional value. But fresh isn’t your only option. Frozen vegetables and fruits can be a nutritious, convenient and healthy alternative. Some canned varieties can also be part of a healthy diet, but be sure to pay attention to added sodium and sugar.

Avoid Ultra-Processed Foods for Heart Health

A key theme in the new Dietary Guidelines for Americans is an emphasis on eating unprocessed foods. This concept receives far more emphasis today than it did in the original food pyramid.

What is Ultra-Processed Food?

Ultra-processed food is broken down mechanically or chemically into components that barely resemble the original item. They are then combined with other ingredients, some of which you would not normally find in a kitchen. Items like emulsifiers, hydrogenated oils, preservatives, artificial coloring, artificial flavors, and sweeteners like high-fructose corn syrup. Some foods are then mechanically processed by extruding, pressing or heating. Common additives include sugars, salt, and fat to intentionally overstimulate the brain’s reward centers and encourage people to eat more.

According to the CDC’s National Center for Health Statistics, over 50% of the food consumed in the United States is ultra-processed.

Examples of ultra-processed foods

Sugary drinks: Soda, fruit drinks, flavored coffee, energy drinks

Breakfast foods: Instant oatmeal, cereal, breakfast sausage

Reconstituted meats: Hot dogs, chicken nuggets, deli meat, fast food burgers, fish sticks

Frozen/instant meals: Frozen pizza, TV dinners, microwave meals, instant soup

Snacks: Chips, candy, cookies, crackers

Many ultra-processed foods contain combinations of refined carbohydrates, added sugars, sodium, and saturated fats while providing relatively little fiber.

The American Heart Association reported in 2025 that higher consumption of ultra-processed foods has been associated in observational research with cardiovascular disease and other chronic health problems.

A Heart-Healthy Way to Follow the New Guidelines

Balance

You don’t really need a pyramid to build a healthy meal. A healthy diet contains a balance of proteins, unprocessed carbohydrates, and healthy fats. Prioritize proteins, but make sure to include a balance of fruits and vegetables, dairy, healthy fats, and whole grains.

Variety

Be sure to rotate sources of protein, fats, and carbohydrates. Different foods provide different nutrients. Some also allow for better absorption.

Healthy choices

Choose lean red meat, fish, poultry, and dairy for protein. Use olive or other non-tropical plant oils rather than relying heavily on butter or animal fats. Choose whole-grain breads, oatmeal (not instant), brown rice, and quinoa for carbohydrates.

Avoid:

Ultra-processed foods. Keep processed foods and saturated fats to a minimum. Avoid added sugars and excessive sodium if possible.

You don’t have to follow the proportions perfectly at every meal. Your overall eating pattern over weeks, months, and years matters far more than a single breakfast or dinner. For help determining the nutritional value of certain foods, you can use a calculator.

Does the New Food Pyramid Change What We Know About Heart Health?

The new recommendations do not change what we know about heart health. But they do change how we look at the foods we eat.

The goal is to improve overall health and reduce chronic disease by changing the way Americans eat. Whole, nutrient-dense foods are the key. This can be done by replacing ultra-processed foods with quality protein, healthy fats, and unprocessed carbohydrates.

Final Thoughts

The food pyramid may have been turned upside down, but heart-healthy nutrition hasn’t been completely reinvented.

The biggest lesson to be learned from the new guidelines is to just make healthy choices. Replace processed foods with whole, unprocessed foods. Focus on quality protein from different sources. Eat lean red meat and full-fat dairy, but limit your saturated fat to less than 10% of your calorie intake.

TL;DRThe food pyramid has changed, and so has what we know about healthy eating. He has taught at Molloy College, New York College of Osteopathic Medicine, and Mount Sinai South Nassau Hospital to name a few. In 1999, Mike founded the American AED CPR Association (AEDCPR), an organization specializing in emergency cardiac care training. Under his leadership, AEDCPR has expanded throughout the United States, and internationally into Europe, Asia, and Africa. Through their network of instructors and innovative website, they have trained over a million people in CPR, AED, First Aid, and Bloodborne Pathogens . Mike helped develop one of the first online learning platforms for the NYS department of labor. He continues to develop new and innovative online tools.
